Registered Nurse RN / Clinical Supervisor / Home Health

Job Responsibilities

The Clinical Supervisor is responsible for directing, supervising and ensuring patient care and services are delivered timely, responsibly and safely. The Clinical Supervisor is responsible for direct and indirect supervision of clinical and patient care personnel.

Additional responsibilities for the Clinical Supervisor include:

Monday-Friday 9am-5pm
Receives case referrals. Reviews available patient information related to case to determine hospice home care needs. Assigns appropriate clinicians to case.
Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist clinicians as appropriate.
Reviews patient’s clinical diagnosis, medications, procedure, level of care and clinical course for accuracy prior to signing Plan of Care.
Assists clinicians in establishing immediate, short, and long-term goals, in setting priorities, and in developing plan of care.

Job Requirements

We are looking for dedicated and professional Clinical Supervisor who are committed to providing exceptional patient care and service. It is also important that you are highly organized and that you demonstrate excellent communication and interpersonal skills.

Specific qualifications for the Clinical Supervisor role include:

Active/Current License as a Registered Nurse in the state of practice;
Valid driver’s license and auto insurance;
Current CPR certification;
Active/current agency health requirements;
Must complete required departmental competencies to validate and maintain competency.
