VISTA's Permanent Placement Division is assisting a healthcare organization that is fast growing and highly dynamic in the recruitment of an Extensivist. You will be joining a platform that enables healthcare systems and providers to become population health leaders. A few details of the employed, full-time opportunity:
* Seeking a BE/BC Extensivist to provide services to the senior patient population across multiple settings
* A partner for building a smarter and better healthcare system for communities by integrating with hospitals, physician groups, health plans and employers
* Advanced clinical model is designed to lower cost and increase care quality to create value for the patient with a focus on inpatient prevention
* Travel locally between care centers daily to treat and manage patients (high-risk and frail) in hospitals, homes and care centers
* Communicate daily and coordinate care plans with covering hospitalist, specialty teams, managers and primary care providers
* See, treat and manage 'skilled' patients in the SNFs according to established protocols. These patients are seen at least twice a week until they remain skilled, which is normally from 1 to 2 weeks
* Supervise, mentor and assist Nurse Practitioners as needed
* Participate in a weekly complex case management multidisciplinary meeting
* Schedule: Monday-Friday 8am - 5pm; 8am - noon rounding in hospitals; on-call schedule as needed
* Low patient census of up to 10 patients per day; 20-40 minute consults per patient
* IM or FM residency training required plus 2-3 years of Hospitalist, Family Practice, or Internist experience preferred, but will also consider graduating residents and fellows
* Salary: starting $200-250K base, plus bonus potential: bed days, clinical performance, low admission rate
* Full benefits, CME reimbursement up to $1500 for courses and conferences, relocation, interview travel fees covered, licensing fees
