Job Description

AMI is hiring to support a project-focused Call Center consisting of a team. The Case Manager and Administrative Assistants will monitor a 24/7 call center for inquiries, consultations, and follow-up calls from Long-Term Care Facilities and redirect questions to appropriate resources. The call center staff are essential team members helping triage calls from long-term care facilities needing COVID-19 outbreak response support or as the first point of contact for enrollment in capacity-building efforts. The call center staff are AMI ambassadors with exceptional customer service skills, impeccable attention to detail, and self-motivated team members who communicate across all team members to facilitate attentive service to our client.

Responsibilities

· Responsible for assisting families and the Long-Term Care Facilities, Skilled Nursing Facilities, and other long-term care settings and staff members with healthcare questions and concerns.

· Provides leadership in directing call center inquiries to appropriate clinical or administrative pathways.

· As a COVID-19 Call Center Team member, you will deliver services following client State Public Health directives. Work will involve inquiries from LTCF/SNFs with confirmed or suspected COVID-19.

· Provide specialized guidance and advice on COVID-19 case and contact management for individual cases and outbreak management for exposure sites.

· Gather and analyze information on outbreaks in settings. Identify needs to resolve outbreaks, supply information, and arrange support to sites to resolve outbreaks.

· Virtually investigate reports of a suspect or confirmed cases of COVID-19.

· Research client state health department guidance documents and relevant policies and procedures to inform practice and disseminate this information to staff.

· Coordinate services across community assets.

· Provide information to clients about general information about COVID-19 and public health interventions in the control and prevention of COVID-19, including disease information, self-isolation, self-monitoring, when/how to seek medical attention, and release from self-isolation.

· Disseminate knowledge of infection control practices and outbreak control measures as necessary.

· Prepare detailed documentation on the case, contact, and outbreak management for exposure sites.

· Work collaboratively with other health unit colleagues, health professionals, and agencies (e.g., physicians, local hospitals, long-term care facilities, community health agencies, workplaces) Utilizes critical thinking techniques to solve problems in a fast-paced environment.

Qualifications:

· BSW degree from a CSWE accredited school required/MSW preferred

· Current licensure as LSW required /LCSW preferred in one or more states in the contiguous United States

· Minimum 5 years of experience in direct or clinical social work or case management in Long Term Care, Skilled Nursing Facilities or Group Home

· The ideal candidate has a mixture of 3 years of experience in community interaction and 3 years of experience in a clinical or community setting.

· Experience in public health or community health preferred

· Experience in emergency or outbreak management an asset/ COVID-19 experience preferred

· Experience in completing health assessments is preferred

· Ability to engage the public and develop/support good relations with local government, community, health industry, and contracted agencies

· A demonstrated familiarity with industry regulations, policies, and procedures (CMS, AHRQ, OSHA, HIPAA, JCAHO)

· Ability to establish and maintain collaborative working relationships

· Analytical, problem-solving, and conflict management skills

· Practical organizational and time management skills

· Ability to work with minimal supervision

· Possess a comprehensive understanding of healthcare information and electronic medical records systems

· Understanding current Pennsylvania State and CDC (Centers for Disease Control) and CDC guidelines, regulations, requirements, and best practices on COVID-19

· Excellent communication and time management skills with the ability to speak publicly (if necessary), communicate needs, and meet deadlines in a changing and flexible schedule

· Highly proficient computer skills, including familiarity with Word, Excel, PDFs, video conferencing (Zoom, Teams), and Outlook.

· Demonstrate written and verbal skills to convey clinical information in terms understood by all

· This role's duties and responsibilities are subject to change based on emergency response and client directives

· Must be able to stand or sit for extended periods.

· Must be U.S.-based.

The job duties are flexible and subject to change dependent upon operational needs.
