Job Description

SUMMARY: Chicago House is seeking a Program Manager for the Transformative Housing
Program, designed to reduce housing instability for Transgender Women of Color living with
HIV. The Transformative Housing Program is a Rapid Re-housing/Transitional Housing hybrid
program that operates within the spirit of the Housing First model. Transformative Housing
participants work with the Transformative Housing Case Manager and Career Specialist to
engage with medical care, pharmacy services, and career services, with the ultimate goal of
being able to financially sustain an apartment prior to program discharge. The Transformative
Housing Program Manager, Transformative Housing Case Manager, and Career Specialist will
work closely together to provide wrap around services to up to 40 clients in the first year.
POSITION DESCRIPTION:
* Ability to supervise a diverse case management team using Motivational Interviewing,
Harm Reduction & Trauma-Informed Care.
* Ability to complete home visits to clients' homes when needed
* Ability to respectfully interact with clients and landlords
* Ability to work cooperatively as part of a team
*Ability to respond to emergencies and be on call as required
* Ability to learn and use database systems
* Ability to support the performance and quality improvement process for the department.
* Ability to assess the needs of service recipients, the ability to meet those needs, and the
ability to assess the legal and policy requirements governing service delivery.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
* Oversee and manage a Rapid Re-housing/Transitional Housing Program for individuals
identified as Transgender Women of Color living with HIV.
* Maintain a small caseload of clients.
* Facilitate intakes into the program and make eligibility determinations.
* Work collaboratively with the care team and Director to determine client graduation
and/or discharge from the program.
* Hire, train and supervise program staff, including completing performance evaluations,
and performance improvement plans when needed.
* Delegate and oversee work assignments.
* Identify unmet staff training needs and provide or identify in-service or outside trainings.
* Support and enhance staff's ability to perform their jobs by teaching and modeling, as
appropriate: technical knowledge and skills, work management and communication
skills, and conflict management skills.
*Address interpersonal barriers and strengths in staff, empower staff, and offer criticism in
a constructive manner.
* Maintain an understanding of employment and labor laws, and seek guidance from
Human Resources when needed.
* Provide weekly clinical/administrative individual supervision, weekly case conferences,
and monthly IDT group case reviews with the care team, including the Career Specialist,
Transformative Housing Case Manager, and Ryan White Case Manager. Support staff in
improving performance as well as preventing burnout.
* Train and support staff in utilizing the agency and department philosophies of care,
including: a Strength's Based approach, Motivational Interviewing, Harm Reduction,
Trauma-Informed Care and a Housing First approach.
* Routinely conduct and follow up on Quality Assurance activities, working with staff to
follow policies on chart documentation and client services.
* Respond to and intervene in client crises, including physical health, mental health,
housing, and other crises as they arise. Report hospitalizations, housing crises, health
and/or behavior changes of clients to the program director.
* Respond to on-call phone outside of business hours and consult with the program Codirector(s) when needed, especially concerning any issues of client suicidality and/or
homicidality.
* Effectively respond to concerns from landlords and clients.
* Ensure that required monthly and fiscal year rent and utility reports are completed
accurately and provided to finance.
* Submit monthly/quarterly/annual reports and other correspondence necessary to report
on activities of program and program participants' progress.
* Participate in program evaluation with Director of Scattered-Site Housing Programs to
ensure that program outcomes and indicators are being appropriately met. Co-develop
and implement Quality Improvement plans to improve service delivery, outcomes, and
client experience.
* Monitor and ensure programs meet contract scopes and objectives, as put forth by
CDPH in grant contracts, in addition to any other funders. Identify scopes at risk of not
being met and implement improvement plans.
* Ensure consistent implementation of policies and procedures towards effective delivery
of housing and case management services and minimum standards of care. Propose
revisions to policies and procedures as needed in accordance with the Council on
Accreditation.
* Assess and identify gaps in program delivery and work to resolve.
* Train, monitor, and support staff's consistent use of associated databases, including the
CHASSA Access Database.
* Collaborate with the Case Management Department regarding client outreach and
medical adherence.
* Collaborate with the Pharmacy Services Manager regarding medication adherence.
* Review applicant eligibility documentation in order to approve new clients prior to their
program enrollment.
* Conduct meetings with clients in the case of housing resource loss and/or non-rent
payment, creating and following up on Supplemental Contracts as appropriate. Schedule
follow up meetings with Program Director for those clients who have already breached
an initial Supplemental Contract.
* Perform other duties as assigned
REQUIREMENTS/QUALIFICATIONS:
* Master of Social Work (MSW) or related field strongly preferred. LCSW/LCPC preferred.
* Bachelor's of Social Work (BSW) or related field required. Bachelor's may be waived
with demonstrated experience, knowledge and effectiveness in homelessness/housing
services with highly vulnerable populations when all other requirements are met.
* Knowledge of chronic medical health issues, including serious mental health and
HIV/AIDS Required.
* Knowledge of and experience practicing Harm Reduction, Trauma Informed Care &
Motivational Interviewing required.
* Experience working with vulnerable populations required.
* Knowledge of housing programs, housing instability, and homelessness services
required.
* Experience working with trans communities preferred.
* Shared identities with program participants preferred.
* Housing and management experience preferred.
* Bilingual / bicultural preferred.
