Job Description

Primary PurposeAssess Senior Care Partners P.A.C.E. participants' functional mobility and components affecting their ability to live independently in the community. This position is responsible for the overall physical therapy program at Senior Care Partners P.A.C.E. This will include providing and recommending therapeutic interventions. Accountability Chart Key Roles Comprehensive assessments in the center and care planning. Provide treatment to improve/maintain function. Provide education to participants and caregivers. Oversee wellness programming. Provide direction and support to Physical Therapy Assistants (PTAs). Duties and Responsibilities Conducts initial and periodic assessments, identifies, and develops a participant centered plan of care for the functional needs of the Senior Care Partners P.A.C.E. participant and assures all are completed as described in Senior Care Partners P.A.C.E. policy to support the organizations mission. Assesses the medical necessity for adaptive and/or durable medical equipment and makes recommendations to the Interdisciplinary Team (IDT). Initiates order for equipment and performs quality checks ensuring its meeting the participant s needs. Plans, develops, and implements a participant-centered plan of therapy to include therapeutic exercise, modalities, and manual therapy. Actively participates in participant centered care planning. Participates in the transition of participants from the hospital, assisted living, and/or skilled nursing facilities. Consults and assists with the Recreational Therapy department in provision of physical wellness programming groups at the center. Provides education and training to caregivers, family members, and friends as needed to support the plan of care for the participant-centered plan of care. Functions as a member of the IDT, communicates participant changes to team members and the development of the participant centered plan of care. Supports and promotes a high level of customer service to our participants, visitors, and internally between departments and staff members. Must care, support, and collaborate with everyone involved in participant s family using the patient-family centered care approach. Initiates and/or participates in the development of new programming for the benefit of participants and that correlates with the organization s strategic plan, vision, and values. Documents in the participant s medical record as required by policy. Responsible for completing all assigned company education and file requirements on time. Participates in Quality Improvement activities. Keeps leader informed of problems or issues. May provide student oversight. Performs other duties as assigned. Leader Responsibilities This position has no leader responsibilities. Qualifications Doctorate in Physical Therapy (DPT) degree required; unless previously grandfathered in under a Bachelors or Masters in Physical Therapy. Must be a graduate of a Physical Therapy (PT) program approved by the American Physical Therapy Association, the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council on Medical Education of the American Medical Association and the American Physical Therapy Association or other equivalent organizations approved by CMS. Must possess a current, unencumbered, active license to practice as a Physical Therapist in the State of Michigan. Minimum of one year of experience working with the frail, elderly, or long-term care population. Ability to collaborate effectively with other IDT members. Detail oriented with good problem solving skills. Strong communication skills, both written and verbal. Well-developed ability to show discretion and maintain confidentiality. Excellent organizational skills. Must be knowledgeable with MS Office Suite, Word, Outlook, PowerPoint & Excel. Specific Requirements Possess current CPR certification by AHA or willing and able to obtain as offered by Senior Care Partners P.A.C.E. Must provide exceptional customer service in support of all staff, external partners and participants. Must possess the ability to make independent decisions when circumstances warrant such action. Must be knowledgeable of current physical therapy practices and procedures, as well as laws, regulations, and guidelines that pertain to .P.A.C.E. Must be willing to seek out new methods and principles and be willing to incorporate them into existing physical therapy practices. Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care. Immunizations and vaccines must be current or medical documentation must be provided from a medical provider with reason (s) to request waiver (Hepatitis B, TB, MMR, Varicella, TDaP, Flu, COVID-19). Must have reliable transportation, current valid Michigan driver s license, and car insurance. Work Environment Has a visible presence while working in the center and is subject to frequent interruptions. Communicates with staff, participants, caregivers, the board of Senior Care Partners P.A.C.E. and other organizations as necessary. Works beyond normal working hours and on weekends and holidays when necessary. May be subject to exposure to TB, AIDS, and/or Hepatitis B virus. May work in an unpredictable home environment with variations in cleanliness, temperature, behaviors, smoke, animals, and other potential hazards. Subject to callback during emergency conditions (e.g., severe weather, evacuation, disaster, etc.). Required to drive a car in all types of weather. Physical Demands (With or without accommodations) Must be able to cope with the mental and emotional stress of the position. Must function independently, have flexibility, personal integrity, and the ability to work effectively with participants, personnel, and support agencies. Must be able to push, pull, move, sit or stand for long periods. Lift up to 50 pounds as needed.
